Qualifications Explained
In the Volunteer Portal, the term "qualifications" is used to describe requirements for volunteering at SJV. Three qualifications (fingerprinting, VIRTUS certificate, and signed volunteer waiver) must be satisfied to gain full access to volunteering opportunities. Below is an explanation of the required documents, whether you are new to the SJV school community or an existing family. These documents are called "qualifications' and must be uploaded to the Volunteer Portal (if you have them) or cleared by Kelly Johnson, Office Manager.
NOTE: When uploading documents for each qualification, please ensure that you are uploading the correct document. An incorrect file uploaded for a particular qualification will delay the approval process for your account, and we will ask you to resubmit the correct document.
Fingerprinting
This qualification is required only once, when you first join the SJV school community. The example shown below is the LiveScan Request form. If you have been fingerprinted for another location within the Diocese of San Jose, please contact Kelly Johnson, SJV Office Manager.

VIRTUS Safe Environment Certification
The VIRTUS Certification (example shown below) expires every 3 years. If you're unsure when your certification expires, visit the VIRTUS website. Certificates can be downloaded from your VIRTUS user account. Your expiration date is 3 years from this date, which is printed on the certificate. Simply upload to the Volunteer Portal under Qualifications and add the expiration date on the submission form. If you do not have a certificate, we encourage you to take the online certification only takes approximately 30 minutes to complete.

Volunteer Activity Waiver Form
This form (example shown below) needs to be renewed every school year by all volunteers. The information required on this form is your medical insurance and emergency contact in case you are injured. If you have not filled out this form, please download the PDF from the school website, complete and sign it, then upload it to the Volunteer Portal.
